Abstract

A workshop on transfer factor, sponsored by the Immunology, Allergic and Immunologic Diseases Program of the National Institute of Allergy and Infectious Diseases, National Institutes of Health, was held in Bethesda, Maryland, on February 25, 1981. The purpose of the meeting was two-fold: (1) to review the state of the art of transfer factor and (2) to suggest future directions for research in this area, specifically in regard to the prophylactic use of transfer factor for varicella-zoster in leukemic children.

TF and TF+ Support in Malignancy in Childhood (Наблюдение доктора Р.Робертсона)
ID is an eleven year old with a complex leukemia history. He was diagnosed with Acute Lymphoblastic Leukemia at age 2 with a relapse several years after chemotherapy. Next came a bone marrow transplant and THREE more relapses after the transplant. To add insult to injury, the last relapse was diagnosed as Acute Myeloblastic Leukemia, a VERY difficult cancer to treat. On to a very toxic course of chemotherapy and a slow definitive road to remission. ID was started on TF initially at month 5 of chemotherapy to help support his immune function and hopefully, reduce the chances of infectious complications. TF+ was added 2 months later. His oncologist (my medical partner) has been very pleased and impressed with the results. First, ID "breezed through his therapy" tolerating very low blood counts, with no febrile (or infectious) episodes, and always in very good spirits. When last seen in early 9/99, ID was "thriving" and continues in full remission, with no infectious disease, enjoying a full life as a vigorous 11 year school kid. The "way Life should be". How has TF and TF+ impacted this young man? It most likely spared him the life-threatening complications of infectious disease. It apparently improved his tolerance of a very toxic course of chemotherapy. And it may be helping him in his daily immune fight against relapse and infectious stresses. Unquestionably, the fact that ID is alive is a miracle, one dictated by a "Higher Authority": the quality of his life may just be because of immune boosting with 4LR products... And his spirit and his will to live.

Стандартное лечение рака предстательной железы III стадии является безуспешным, выживаемость больных в этой стадии плохая.

Имеются предположения, что гуморальный и клеточный иммунитет(CMI) при раке простаты имеют отношение к опухолевым антигенам (TAA).

Эти наблюдения побудили нас применить в лечении больных с раком простаты ТФ. В этом исследовании принимало участие 50 больных раком простаты III стадии. Им ежемесячно вводилось 2-5-единиц внутримышечно специфичного ТФ. Наблюдения, продолжавшиеся от 1 года до 9 лет, показали, что полная ремиссия наступила у двоих, частичная ремиссия у 6, сдерживание метастазирования - у 14 больных. Средняя медиана выживаемости составила 126 недель, что выше, чем у больных этой стадии по данным литературы.

A group of 67 patients with head and neck cancer has been studied of which 40 have received immunologic transfer factor from a normal donor pool. Examination of these patients revealed that lymphocyte reactivity to nonspecific mitogrens is depressed in patients who have head and neck cancer to a much greater extent than is seen in patients with other types of tumors. Furthermore, the depression is more prevalent among patients who have been treated with radiation. Patients in the head and neck group who have received transfer factor show an initial decreased response to PHA stimulation in culture. This is not seen in a control group of head and neck cancer patients or in patients with nonsquamous cander. Thymus-derived lymphocytes are depressed in patients with head and neck cancer, irrespective of whether they have received radiation. Th T-lymphocyte levels increased in eight of 38 patients who received nonimmune transfer factor, but 7 of these were in the group who had not received radiation. The leu kocyte adherence inhibition (LAI) test has been used to determine tumor immunity in the patient test group. Changes in tumor immunity did not occur in those patients who received normal nonimmune transfer factor. Studies are presently in progress which provide for treatment of patients with head and neck cancer with specific squamous carcinoma immune transfer factor.

Epstein-Barr-Virus (EBV): Immunomodulatory effect of Transfer Factor (TF) in Nasopharyngeal Carcinoma (NPC): A pilot study

Abstract

EBV is strongly associated with the etiology of NPC. Strengthening the host-defence against this virus is considered to play a role in controlling NPC at advanced stage, besides the standard therapy. TF, with specific activity against EBV has shown favourable results when used in patients with Burkitt's lymphoma, another cancer associated with EBV. A pilot study was undertaken to see whether the same TF will have impact in NPC. Out of 12, all male, advanced stage NPC patients, 6 were treated with TF (group1) in addition to standard therapy, while the other 6 (group2) received non-specific therapy. Immune response was monitored by skin-window test. After a median follow-up of 61 months, the median survival for the group 1 was 44.5 months as against 19 months for group 2 (P = 0.025). None in group 2 could survive, while in group1, 3 are surviving, out of whom 2 had bony metastasis to start with. Cellular responses to autologous NPC tissue very well commensurated with the result.

Вирус Эпштейна-Барра (ВЭБ) тесно связан с этиологией назо-фаренгиальной карциномы (NPC). Укрепление защиты хозяина против этого вируса, помимо стандартной терапии, как полагают, играет роль в контролировании NPC на продвинутой стадии. Применение трансфер фактора со специфической активностью против (ВЭБ) показали благоприятные результаты, при использовании больных с лимфомой Беркитта, другого рака, связанного с ВЭБ.

 

Было предпринято экспериментальное исследование, чтобы

установить, окажет ли такой же эффект ТФ у больных NPC.

 

Из 12 больных (все мужчины) с продвинутой стадией NPCшесть больных получали ТФ (1 группа), в то время как другие получали неспецифическую терапию. Иммунный ответ изучался по кожному тесту.

This review has attempted to describe the characteristics of transfer factor which make it a very attractive potential agent for immunotherapy. Preliminary observations suggest that it may be capable of modifying resistance to a variety of diseases including cancer but considerable progress in basic knowledge regarding this agent is crucial to its successful application in clinical disease states. Fortunately, a sizable number of interested and dedicated investigators are exploring these difficult problems and their success may lead to new approaches in immunotherapy.
